By booking online or over the phone or in person, you automatically agree to accept all OM Spa Polices and Terms.
If you want to book outside of regular Om Spa business hours, please let us know in advance and we will try our best to accommodate you as soon as possible.
If you would like to book an appointment on a date with no open availability, please contact SPA to check potential additional availability.
For your best experience with OM SPA, your safety and treatment’s best results, please, check all treatment(s) pre-care – preparation and after procedure care here as well read all Om Spa terms, Conditions and Policies, BEFORE booking an appointment and making payment(s), because the booking fee are nonrefundable. Please, read all information below.
APPOINTMENTS TERMS AND CONDITIONS:
We do work by appointment only. SMP appointments, Areola Nipple reconstruction and paramedical procedure require a consultation. *Clients with previous PMU done by other artist must contact the studio prior to booking an appointment.
YOU MUST BE 18 years of age or older to enter our premises, schedule appointment and have a service done. We do not perform cosmetic tattoo on minors even with parent’s and physician consent. If you are in doubt as to whether tattooing safe for you due to your current health condition, please consult your doctor for written consent for the procedure.
You must present a valid state ID/DL or passport (by Health Department requirement ID documents cannot be write in Chinese, Arabic or any other language that is impossible to read /understand with regular alphabet letters; also date of birth as the proof of being 18yrs old must be clear). ID must be shown with every visit, before each tattoo session (including pigment removal, microneedle/dry tattoo) . Om SPA, LLC keep rights to refuse service if no valid ID is present, and you will forfeit your booking fee and you will be responsible to pay 100% treatment price. Also, to book another appointment we will require another booking fee.
If you book an appointment and upon arrival you will change your mind and want to cancel, you will forfeit booking fee and will be responsible to pay remaining balance in full.
PLEASE NOTE: All face/scalp/ permanent makeup requires a minimum of two to three appointments. Your mandatory permanent makeup follows up (called also touch up) session will be booked withing 8 to 10 weeks of your initial visit. To ensure that your follow up occurs withing the allotted time frame the appointment will be scheduled at the conclusion of your initial appointment. If you miss your booked touch up or need reschedule outside of the 8 to10 week period, your new appointment will be subject to additional fees (varies according to artist) – NO EXCEPTIONS.
Majority of all body cosmetics tattoo camouflage (scars, discolorations, stretchmarks, full head hair micropigmentation) usually will require multiply sessions (depends of the complexity of the work- this will be priced upon consultation as in a package).
We provide the same high quality service for every client, but the healed results can vary with every individual person due to pigment retention and other factors like: your immune system/body metabolism, skin type and its condition, any medications /supplements taken, life style (sun bathing and tanning, sweating, swim, skin care regiments, etc.), following strictly our instructions for before procedure and aftercare. Therefore, healed results vary and are beyond our control. If your ink retention is low, more session might be necessary to achieve desirable results, and that will require to schedule new appointment and extra fees of $250 will apply (including booking fee) for each treatment. Additional touch up will be required withing 8 to 10 weeks after your appointment to achieve the desired results, since this will be the same as doing a new procedure.
In depth consultation and mapping with précised measurement and color selection is the integral part of every permanent makeup service we provide, and it is included with the treatment price. You will be asked to approve shapes, colors and position of your new makeup.
We suggest you bring your favorite colors/makeup with you so it will be easier to match. You might come with your normal, NOT WATERPROOF makeup.
If you are looking for more in-depth in person additional consultation and face/skin/color analysis, please, schedule additional 30 minutes paid consultation, before coming to your major treatment. At this time, we will interview you about your lifestyle, preferences, skin /hair regiment, and medical conditions and taken medications /supplements.
Advise and makeup design will be demonstrating on your face, based on your personal beauty type, style/ goals and your budget. Ask all questions and get all the right answers. The fee for this service is not credited towards your procedure.
Upon client request we might also provide consultation over the phone, email, or WhatsApp and Messenger however in person consultation is more accurate and valuable for both client and technician. Also, we highly recommend adding on to this appointment a Color Patch Trial Skin/Skin Sensitivity Patch Test. It can help to determine skin sensitivity, healed pigment outcome and client’s skin undertone/color temperature. It is done only upon your request. This test is not intended as an allergy test. Consult your doctor if you are prone to an allergy or you already had one, for written consent letter for cosmetic tattoo, BEFORE procedure.
FOLLOW UP (CALED TOUCH UP ) and COLOR BOOST:
One follows up /Touch up / at Om SPA is mandatory and is NOT INCLUDED withing first appointment; it will be scheduled after the conclusion of your initial appointment withing 4 to 10 weeks. It is designed to use the same color, technique and size/thickness.
First follow up and every additional “perfecting” session is charged additionally $250 (except corrective work after other technician) for each booked procedure.
Color Booster /Color Refreshing Session is an appointment done after minimum twelve months to four years, for the purpose of restoring freshness of the originally inserted pigment (which is most likely faded by this time). Please note, that you mut have at least 60% of the color remaining in the skin to be qualify for the Follow up or Color Boost. More loss than that will require you to schedule new/full set procedure. To avoid higher ticket price in the future we highly recommend booking your color boost appointment right after your follow up (ask for special offer).
CHANGING YOUR TECHNIQUE/ COLOR /SHAPE: If you decide to do /upgrade / to another technique/style, change a shape, thickness or color on your follow up or annual color boost appointment, extra $250 will be applied. Also, if there is necessary to book third visit (additional “perfecting” session), the extra fee will be applying accordingly to the artist pricing.
PERMANENT MAKEUP FROM ANOTHER ARTIST/COVERUP CORRECTION: If you had your permanent makeup done by another artist and are interested in a cover up /correction/ rework, you must first get approval by sending us photos of the existing work.
If you fail to do so, we have the right to refuse or add additional charges to the service on the day of your appointment. As a result, your booking fee will be forfeited. Not all previous tattoos from other artist can be fixed or covered. Corrective work usually requires multisession and patience (at least three visits if not more) and is priced as a package only, upon individual consultation.
RIGHTS TO REFUSE SERVICE POLICY:
Om SPA reserve rights to refuse service to anyone for any reason at any time. For our client’s safety and achieving best ink retention, we reserve rights to interrupt the service when we notice, that you/ and /or your body doesn’t handle tattooing well and we believe, that it may not be safe to continue / to prolong with the procedure, for instance: if excessive bleeding, bruising, puffiness or client’s extreme sensitivity will occur at that day; at that scenario we will end the procedure and you will be asked to book new appointment with additional $200 fee. Due to all above factors WE DO NOT REFUNDS – NO ECXEPTION.
OM SPA PREMISES POLICY:
NO KIDS, NO FOOD ARE ALLOWED inside the treatment area. Please be respectful and understand that PMU artist must focus on the permanent tattoo procedure. We also want to ensure the safety of our staff & clients. If you are sick or have any flu-like symptoms, please reschedule your appointment. If you show up to your appointment with any flu, cold, etc., like symptoms, OM SPA has the right to refuse service; you will be asked to reschedule appointment with new booking fee if rescheduling won’t happen withing less than 72 hours. NO EXCEPTIONS.

PAYMENTS AND PRICE POLICIES:
We accept Visa, Master, AMEX, Discover – there will be extra 3% convenient fee and for invoicing service apply to all credit cards payments done when choosing any payment plan /or Membership we offer.
You might pay with cash, or ZELLE transfer to our business cellphone 773 641 3373 as a form of payment (to avoid convenient fee).
Your balance is due in full on the day of your procedure.
We accept only Om SPA gift cards.
We DO NOT accept PERSONAL CHECKS nor medical credit cards, except Care Credit.
We do not bill to an insurance / nor accept any. We might issue an invoice per your request to submit to your insurance.
NO REFUNDS -NO EXCEPTIONS
BOOKING FEE must be done to secure your date (you will be asked for credit card information). Appointments are not kept on hold without that.
For all permanent makeup initial appointments on eyebrows, eyeliners, lips, corrective pmu work and medical tattoo services there is $100 booking fee for each procedure you are going to schedule.
To book first follow up for face permanent makeup $100.
Corrective work after other technicians along with para medical tattoo are priced upon consultation according to complexity of the work; multisession might be required to achieve desirable results depending from complexity of the work. Please, contact office for pre-qualification.
Scalp Micropigmentation– booking fee is not less than $500 (except if client wants to prepay all in full)
OM SPA reserve rights to change prices at any time without previous notice. All booked and pre-paid in full appointments will be honored as paid.
YOU MUST BE A CARD HOLDER or authorized person to make payments and to book appointments on somebody’s behalf.
TO BOOK APPOINTMENTS: we require NON-REFUNDABLE booking fee. All booking fees will be applied toward your balance on the day of your appointment. NO EXCEPTIONS. You will be asked for credit card at that moment to secure your spot.
Appointment can be booked, rescheduled or cancelled in different ways:

– we do not honor cancellation requests via any social media platform.
If you need to reschedule your appointment, you may do so by emailing us or by calling any time up to 72 hours (3days) before your scheduled appointment, otherwise you will lose your deposit and additional fee will be required to make a new appointment reservation. NO EXCEPTIONS, as you can appreciate both your time and our time are very valuable. If you reschedule more than one time, you will need to pay the entire treatment in advance. Payments are NON-REFUNDABLE AND NON-TRANSFERABLE- NO EXCEPTIONS
LATE ARRIVALS POLICY:
Please, if possible, arrive 5 to10 minutes prior to your scheduled appointment or at least be on time for filling up paperwork (except if not signed up electronically prior to the visit). If you are more than 15 minutes late, Om SPA, LLC reserve the rights to cancel your appointment, and your booking fee will not be refunded – regardless of unforeseen circumstances that might arise, which could force you to postpone /reschedule your procedure.
All appointments must be cancelling or reschedule UP to 72 HOURS (3 days).
For the last-minute booked appointments (withing less than three days)- client is giving up on his/her rights to cancel or reschedule appointment withing up to 72 hours. If the appointment is cancelled or rescheduled withing less than that time frame, client will forfeit the booking fee, and new fee will be required to schedule new appointment. If you reschedule appointment more than two times, you will be asked to pay entire treatment in advance. Payments are NON-REFUNDABLE and NON-TRANSFERABLE – NO EXCEPTIONS.
Late cancellation withing less than 72 hours and /or NO SHOW will get charged 100% of the scheduled service price. NO EXCEPTION. If the card on file for your returns as “Insufficient Funds”. We will attempt to process for 30 days. If payment is not secured after 30 days OM SPA reserves the right to pursue legal action.
All appointments scheduled as a “MODEL FOR A STUDENT/ an apprentice”) are subject to cancellation withing not late than 7 business days. In the case of cancellation, the booking fee will be forfeit and new booking fee will be required to make a new appointment. Om SPA, LLC does not guarantee, when the next appointment will be available, because these appointments have very limited space and date availability and are first come – first serve. Models should have not done any permanent makeup except if agreed differently.

We accept all major credit cards: Visa, Master, Discovery, and Zelle transfer.
We might offer a payment plan for our cosmetics tattoo services and TRAININGS; if applicable, client must be pre-qualified for that type of service. To register /booked a date a required fee must be completed at the registration /booking.
After that, payments might be done weekly, biweekly or every month for the period up to 24 weeks ( 6 months). All payments must be completed 14 business days BEFORE your training start date /appointment. There will be additional 3% credit card convenient fee added to every payment and for invoicing service. All payments are nonrefundable and nontransferable because all services have very limited space and dates are book at first come, first serve. NO EXCEPTION.
ALL SALES ARE FINAL. NO REFUNDS OR EXCHANGE POLICY ON ALL OM SPA SERVICES, GIFT CARDS, TRAININGS, BOOKING FEE – NO EXCEPTION (including Covid-19 and other unforeseen life circumstances).
OM SPA STATES NO MONEY REFUNDS for all provided services, due to the nature and risk involved with specific treatments factors ( Appointment Terms and Conditions Policy) and fact, that we are working with human body, and that everybody’s reaction to tattooing is very unique and unpredictable. If you are unhappy with a service that you received at our place of business, we will work with you to correct the problem to ensure your satisfaction.
We will refund money for all skin care products purchased at the Spa location only and had not been opened or used and are in original package. You can return it withing 30 days with valid and easily readable, undamaged receipt.
GIFT CERTIFICATES POLICY:
Gift Certificates can be used towards all OM SPA services or purchase of skin care.
NO TIPS ON GIFT CARDS (tips only in cash or on credit cards).
There is no cash back or cash/money refund over any purchased Gift Certificates.
Gift Certificates has no cash value.
All lost/or stolen Gift Certificates will not be replaced.
Gift Certificates has no cash value.
LOST/STOLEN PERSONAL ITEM:
Please leave all jewelry and valuable items at home. OM Spa is not responsible for lost or misplaced items. We regretfully cannot be responsible for any loss, theft or accidental damage to your personal belongings so please take extra caution to prevent any such occurrence. Please do not wear your best clothing for the appointment, as we are working with pigments and clothing can be stained.
WEBSITE TERMS AND POLICY

REFUSAL OF SERVICE
The Services are offered subject to our acceptance of your order or requests. We reserve the right to refuse service to any order, person or entity, without the obligation to assign reason for doing so. No order is deemed accepted by us until payment has been processed. We may at any time change or discontinue any aspect or feature of the Site or Service, subject to us fulfilling our previous responsibilities to you based on acceptance of your payment.
ORDER CONFIRMATION, CONSUMER RETURN & REFUD POLICY
Om spa offers online skin care products purchase via drop shipping from our vendors. We are not responsible for return packages that may be lost or damaged in transit.
Upon order via website, you should receive automatic order confirmation. If there is an error in the order, that client made, it is your responsibility to inform us as soon as possible to correct; we will try to correct your order if still possible client is still responsible for full payment. We might exchange unwanted product for different if available only at the spa location.

We only accept the return of products originally purchased through the www.omspachicago.com website or at the Om Spa location.
Returned items are subject to inspection. All printed materials, promotional items, accessories, and any original product packaging must be returned. Returned products may not be unopened, unused not altered or damaged.
Repeated return requests are subject to review. We reserve the right to deny multiple refunds to an individual who has repeatedly returned products.
Returns that do not meet the return policy criteria will not qualify for a refund and may be returned at the customer’s expense or destroyed upon request.
We do not accept any product returns, exchanges, or refunds after 30 days from the date of purchase. We are not taking any responsibility for skin sensitivity (reaction) that occurs due to products purchase at Om Spa; there won’t be a refund or exchange -NO EXCEPTION
We endeavor to describe and display the Products and Service as accurately as possible. While we try to be as clear as possible in explaining that, please do not accept that the site is entirely accurate, current, or error-free. From time to time we may correct errors in pricing and descriptions. We reserve the right to refuse or cancel any order with an incorrect price listing.
REGISTRATION AND SECURITY

MERCHANDISE, PRODUCTS, AND / OR SERVICES

DELIVERY AND RIGHTS REGARDING ORDERS:
The estimated delivery time should be specified in your order; however, as we use a third party to sell and deliver orders, it is possible that your order will arrive before or after the estimated delivery time. Because we sell product based on “drop off” method, we will not be responsible for any delays in delivery which are beyond our control. If you do not receive your order within 15 days of the estimated delivery time, please let us know by emailing . Please note that multiple orders may not always be shipped together. In addition, depending on the size of your order you may receive several shipments to complete your order.
The tracking number and estimated delivery time might be/or might not be specified in your Order; however, as we use a third party, and different vendors ( drop shipping) to deliver Orders, it is possible that your Order will arrive before or after the estimated delivery time. Information (including, but not limited to, information relating to product descriptions, pricing, promotions, offers, and/or availability) provided by the OM Spa, LLC and its vendors, and/ or third parties. affiliates in relation to the Websites are believed to be accurate at the time of publication. Unfortunately, however, there may sometimes be information on the Websites that contains typographical errors, inaccuracies, or omissions. Verification of Orders
All Orders are subject to verification by the Company at any time and for any reason. We reserve the right, in our sole and absolute discretion, to require proof of identity (in a form acceptable to us): (i) for the purposes of verifying the legitimacy of any Order and/or other information; and/or (ii) for any other reason we deem necessary, in our sole and absolute discretion, for the purposes of fulfilling an Order in accordance with our interpretation of the terms and conditions of this Agreement.
